How to Submit Your Annual Waiver

Important: Completion of this waiver is mandatory for all members.

You may submit your waiver using one of the following options:

Option 1: Download, Print, and Email

  1. Download the waiver
  2. Print the form
  3. Sign it
  4. Scan it back to your computer
  5. Email the scanned copy to SONCmembership@gmail.com

Option 2: Complete Online and Email

  1. Fill out the waiver online
  2. Save the completed PDF
  3. Email the saved PDF to SONCmembership@gmail.com

Option 3: Complete Online and Bring In Person

  1. Fill out the waiver online
  2. Print and sign the form
  3. Bring it to your next meeting or event

Option 4: Complete In Person

  1. Fill out a paper waiver at your next meeting or event
  2. Copies are usually available, but bringing your own is recommended
